Nhận 2 ca lâm sàng hội chứng Budd-Chiari

Bài báo này mô tả 2 trường hợp bệnh nhân được chẩn đoán hội chứng Budd-Chiari. Một bệnh nhân nữ 56 tuổi, biểu hiện hội chứng Budd-Chiari bán cấp bởi tình trạng cổ trướng và tăng men gan, bệnh nhân được quản lý thành công bằng thuốc chống đông và lợi tiểu. Một bệnh nhân nam 39 tuổi, biểu hiện bệnh mạn tính bởi cổ trướng, xuất huyết tiêu hóa do tăng áp lực tĩnh mạch cửa ba lần và có xơ gan. Việc quản lý bệnh nhân hiện tại đơn thuần là kiểm soát biến chứng của xơ gan và điều trị chống đông để phòng ngừa các biến cố tắc mạch có thể xảy ra, do bệnh nhân chưa có điều kiện kinh tế để ghép gan. Huyết khối là nguyên nhân chính gây tắc mạch gan, cả hai bệnh nhân này đều phát hiện có các đột biến gen đồng hợp tử của nhóm gen làm tăng nguy cơ huyết khối. Đối với mỗi bệnh nhân, việc điều trị thuốc chống đông là điều cần thiết, tuy nhiên phẫu thuật giảm áp lực tĩnh mạch cửa hoặc ghép gan còn tùy thuộc vào tình trạng lâm sàng và điều kiện cụ thể của từng bệnh nhân.
